Covid Vaccination Policy

September 14, 2021

To our patients,

Due to increased transmission of Delta variants, San Jose Gastroenterology (SJGI) has issued a new policy that effective November 1, 2021, SJGI will no longer schedule in-person visits for unvaccinated patients. Unvaccinated existing patients on the schedule are provided ample time to transfer care to another physician or show proof of vaccination in order to keep the appointments. Depending on your doctor's availability, unvaccinated patients may opt to convert your clinic appointments to telehealth visits. If you were vaccinated in California, you can get your digital copy of your vaccine records at https://myvaccinerecord.cdph.ca.gov.

You can email a copy of your vaccination card or the digital copy to contact@sjgi.com. You can also show proof of your vaccination on the day of the appointment. Failure to do so may result in the appointment being cancelled.

Most vaccinated patients do not need a Covid test. But if you have been exposed to Covid-19, have symptoms, or traveled internationally within 10 days of your appointment, you must get a Covid test.

Please note that we do not provide Covid testing at our clinic.

Thank you for your cooperation.
